Direct Primary Care vs Traditional Insurance: What’s the Real Difference?

July 22, 2026

About Me

I'm Dr. Berry — board certified family medicine physician and breastfeeding medicine specialist.

Meet Dr. Berry

If you’ve been researching Direct Primary Care vs traditional insurance, the two get confused constantly, and it’s an easy mix up to make. Both involve a monthly payment. Both are supposed to help you get care. But they’re built to do two completely different jobs.

The biggest difference comes down to what each one is actually designed to cover.

Traditional insurance is built to protect you from financial catastrophe. Direct Primary Care is built to give you real, ongoing access to a doctor for everyday care. Most families end up needing both, not one instead of the other.

If you’re trying to understand how they actually fit together, here’s what you need to know.

Why This Question Comes Up So Often

I get some version of this question constantly: “Isn’t Direct Primary Care just insurance with a different name?”

No. And the mix up matters, because if you don’t understand the difference, you might assume you have to choose one or the other. You don’t.

What Health Insurance Is Actually For

Health insurance exists to protect you from financial catastrophe. The car accident. The cancer diagnosis. The surgery you didn’t see coming. That’s what insurance, at its core, was designed to absorb: the rare, expensive, unpredictable event.

Somewhere along the way, we started routing everyday care through the same system. Your annual physical, your kid’s ear infection, your blood pressure check. All of it gets billed through insurance, which means all of it gets tangled up in networks, copays, deductibles, and prior authorizations before you ever see a doctor.

What Direct Primary Care Is Actually For

Direct Primary Care is a membership. You pay The Village one flat monthly fee, and in exchange, you get real access to me: same or next day visits, 30 to 60 minutes of actual time, a direct text line, and no copay or surprise bill for anything we do together.

There’s no insurance company between us. No claim gets filed. No one is deciding what counts as medically necessary before I’m allowed to help you. It’s just you, me, and whatever’s actually going on.

Insurance and Direct Primary Care aren’t competing for the same job. One handles the everyday relationship. The other handles the emergency you hope you never need.

Is Direct Primary Care a Replacement for Insurance?

Not exactly.

DPC isn’t a replacement for insurance, and I’m upfront about that with every new patient. It doesn’t cover a hospital stay, a surgery, or a specialist referral. What it does is take almost everything else off insurance’s plate, which is usually most of what a family actually needs in a given year.

The families who do best with this setup pair DPC membership with a high deductible health plan or a health share. You get low cost, high access primary care every month, and you keep a safety net for the big stuff sitting quietly in the background, hopefully never needed.

Common Questions About Pairing DPC With Insurance

Do I still need health insurance if I join?

Yes.

Direct Primary Care covers your everyday primary care, but you’ll still want coverage for a hospital stay, a surgery, or a specialist referral. Most of my members pair the membership with a high deductible health plan or a health share.

Does Direct Primary Care bill my insurance?

No.

I don’t bill insurance for our visits at all. Your monthly membership covers our care together directly, no claims, no copays, no surprise bill weeks later.

What does DPC actually cover day to day?

More than people expect.

Sick visits, chronic condition management, preventive care, medication questions, and a direct text line between visits. It’s built for the everyday relationship with your doctor, not just the once a year checkup.

What kind of insurance pairs best with DPC?

Usually a high deductible health plan or a health share.

Since DPC handles most of your everyday care directly, you don’t need a plan with a low deductible built for frequent office visits. You just need something to cover you if something bigger happens.

What This Looks Like Day to Day

Practically, it means when your kid wakes up with a fever, you’re not weighing whether it’s worth a $150 urgent care copay. You text me. When you’ve been putting off that nagging question about your blood pressure or your sleep, you’re not saving it up for a rushed annual physical. You just ask, in a visit that has room for the actual conversation.
